Health & Phys Ed Opt-Out Form

It is our hope that our community waits until August to view the actual curriculum that will be taught come 2023-2024, however whether it be now or later, this form will allow parents to opt-out of the specific Sexual Health & Family Life Curriculum of the 2020 Comprehensive Health & Physical Education Curriculum.
