Vaccine Requirements for Children Entering 6th Grade in September 2020
    In accordance with the State of New Jersey regulations, as specified in Chapter 14, Immunization of Pupils in Schools (N.J.A.C. 8:57-4.1 - 8:57-4.20), all incoming Sixth Grade students must receive the following additional immunizations BEFORE entering school in September 2020.
    1- One dose of Tdap (tetanus, diphtheria, acellular pertussis)
    Exception: Children who received a Td booster less than five years prior to September 2020 shall not be required to receive a Tdap dose until five years have elapsed from the last DTP/DtaP or Td dose.
    2- One dose of meningococcal vaccine - routinely recommended for 11 years and older.
    With summer vacations and busy physician schedules we encourage you to make arrangements as early as possible for your child to have their physical examination and immunizations. Please have your physician review your child's immunizations to ensure that they meet the State requirements. After your child receives their vaccinations please obtain documentation from your physician that can be given to the school nurse.
    Proof of the immunizations must be presented to your child's school nurse prior to the first day of school. Your child will be unable to attend the Haines 6th Grade Center without evidence of the proper immunizations.
    Exception: If your child will turn eleven years of age after the first day of sixth grade (September 2020) please call the school nurse.
    If you do not have insurance or your insurance does not cover immunizations please contact the school nurse for further assistance.
