    INHALERS
     
    If your child uses an inhaler they must have an Asthma Treatment Plan on file in the nurse's office documenting its use during school hours, after school activities and during sports when the nurse is not present. This plan must be renewed every year by the physician and parent.  If your child is playing a sport he/she must carry their inhaler in their sports bag at all times as they may be practicing or playing after the nursing staff leaves for the day.  They also may leave Haines/Memorial grounds for games and there is no traveling nursing staff. Students who do not play sports may keep their inhaler in the nurse's office.  ANY STUDENT WHO CARRIES THEIR INHALER IN THEIR BAG  MUST HAVE PERMISSION SIGNED OFF BY THEIR PHYSICIAN  TO SELF ADMINISTRATE.
     
     
     
     
     
    EPINEPHRINE INJECTORS
    ( EpiPens, Twinjects, AUVI-Q)
     
    If your child has an epinephrine injector for life threatening emergencies, an Anaphylaxis Individual Emergency Care Plan must be on file in the nurse's office.  This form must be renewed every year by the  physician and parent. If your child is playing a sport he/she must carry their injector in their sports bag at all times as they may be practicing or playing after the nursing staff leaves for the day. They also may leave Haines/Memorial grounds for games and there is no traveling nursing staff.
     
     
     
     
      
     
     
     
     
     ADMINISTRATION OF MEDICATION WHILE IN SCHOOL
     
    Please read the attach letter concerning the administration of medication (prescription or over the counter) while your child is in school.
     
     
     
     
     
     
     
     
    CONCUSSIONS
     
     
    If your child sustains a head injury that requires accomodations in their academics and /or physical activity levels, please have your physician complete the  attached  form.
